Significant effects, but trial quality is questioned

Background
Turmeric’s active compound, curcumin, is one of the better studied natural therapies for depression. It’s thought to work through anti-inflammatory and neuroprotective effects, and has additional benefits in cognition and arthritis.
The Study
- Nineteen randomized controlled trials, pooling over 1,600 patients across studies on clinically diagnosed depression and separate studies on depressive symptoms in medically ill populations.
- Curcumin (conventional or nano-formulated) compared with placebo, doses ranging from 80 mg to 3,000 mg a day.
- Treatment lasted 4 to 24 weeks, with depression scores tracked on scales like the Beck Depression Inventory (BDI) and the Depression scale (PHQ-9).
Results
Curcumin outperformed placebo for depression (effect size 0.76) and for depressive symptoms in medically ill patients (effect size 0.53), a moderate-to-large benefit.
Curcumin looked better in patients over 45, in trials lasting eight weeks or more, and at lower doses (under 1,000 mg a day). But the significant effects were driven mostly by low-quality studies. When the authors restricted the analysis to high-quality trials, the effect on depressive symptoms lost significance.
Limitations
Heterogeneity across studies was substantial (I² in the 85-95% range), meaning the trials didn’t agree with each other much. Many studies were small, several lacked full blinding, and the underlying evidence was rated low quality by GRADE. Funnel plots looked asymmetric, raising the possibility of publication bias, even though formal tests didn’t confirm it.
Side Effects
Reported side effects were mild: digestive upset, skin issues, and occasional visual or neurological complaints. No serious adverse events turned up in any trial, but there are rare reports of liver injury (even though curcumin has hepato-protective effects as well).
Drug Interactions
Curcumin may increase drug levels by inhibiting CYP1A2, CYP2C9, CYP2C19, CYP2D6, and CYP3A4. These interactions are theoretical and were not confirmed in the few clinical studies that investigated them. Curcumin impairs iron absorption in animal but not human studies (Lainé F et al, Fundam Clin Pharmacol 2017;31(5):567–573).
Medical Benefits
Though not discussed in this analysis, curcumin may improve arthritis, diabetes, dyslipidemia, hypertension, fatty liver disease, ulcerative colitis and Crohn’s disease, acute COVID (also reduces inflammation in long COVID), hot flashes, premenstrual syndrome, male fertility, indigestion, dry eyes, weight loss, and prevention of cancer and dementia. I’ve seen more robust effects on arthritic pain than depression in practice.
Practice Implications
- Consider curcumin in older adults and those with medical disorders that might benefit from it, like arthritis. It may also work better in depression with inflammation (hs-CRP 3 mg/L or greater).
- Curcumin has poor oral bioavailability, which may explain some of the inconsistencies in trials. To improve absorption, use a products with bioperine and take with a large meal.
- Look for products labeled BCM-95 or 95% curcuminoids. I’ve listed lab-tested products here.
