Background
More than half of adults with bipolar disorder carry excess weight, and recent reports suggest GLP-1 meds can raise lithium levels, particularly semaglutide. In June 2026, European regulators started tracking this interaction.
The Study
- The authors reviewed six published cases of lithium toxicity or elevated levels tied to starting, switching, or increasing a GLP-1 drug.
- They sorted the evidence into three tiers: the raw case reports, the biological mechanisms that might explain them, and a speculative theory about timing.
- They tested each proposed mechanism against the pharmacology literature.
Results
One possible mechanism is dehydration, as patients eat and drink less on GLP-1 agonists.
However, one well-documented case showed a lasting rise in lithium level per milligram of dose, persisting for months without any nausea, vomiting, or reduced intake. Weight loss lowers lithium clearance, but the math only accounts for about a quarter of that shift.
What about delayed stomach emptying? This is often blamed, but the authors found it unlikely. Pharmacokinetic data show it changes the timing of absorption, not the total amount absorbed.
Practice Implications
- When you start or increase a GLP-1 drug in a patient on lithium, check a lithium level about a week later and again after any dose change, using the same timing and formulation each time.
- Ask about appetite and fluid intake at that visit, since reduced intake is the likely driver of sudden spikes.
- If a patient stops the GLP-1 agonist after lowering their lithium dose, recheck the level, since they may now be underdosed.
- There’s no reason to avoid lithium in these patients or lower the dose preemptively, as this is an emerging and rare signal, not a settled interaction.
