For ADHD, children with low omega-3 levels respond; the rest may not

Background
Omega-3’s are backed by large, positive trials in:
- Major depressive disorder
- Mild cognitive decline in older adults
But their data in ADHD is mixed. This analysis asked whether a subgroup with low baseline levels responds better.
The Study
- Seven randomized trials in children with ADHD, pooling 10 estimates that measured baseline omega-3 levels in blood.
- Compared EPA and DHA supplements against placebo, mostly for 12 to 24 weeks.
- Outcomes included ADHD symptom ratings (ADHD-RS, Conners’ scale), plus cognitive and academic measures.
Overall, omega-3 barely beat placebo (effect size = 0.21), a small effect that disappeared once the researchers accounted for trials contributing more than one estimate.
But the benefit was more robust in children who started out with low omega-3 levels (effect size = 0.52), while those with normal or unmeasured levels showed none (SMD = 0.03). A separate analysis found effect size shrank as baseline EPA rose.
Practice Implications
- We need more studies like this, and perhaps it’s time to check baseline omega-3 levels.
- But these tests aren’t for everyone. In depression (adults), I only know of one study that looked at baseline levels, and it found the opposite: Normal omega-3 levels predicted response.
